Jadav Form Lead Qualification is a lightweight form builder for creating responsive forms and managing submissions inside WordPress.
Key features:
- Admin form builder with text, email, phone, number, textarea, dropdown, radio, checkbox, date, time, URL, file upload, and page break fields.
- Multi-step frontend forms with progress indicators.
- Shortcode embedding with
[jadav_form_lead_qualification id="123"]. - Saved entries with admin detail view, read/unread state, search, and CSV export.
- Email notifications for every submission.
- Honeypot and one-minute per-IP rate limiting.
- Optional login requirement, open/close scheduling, and entry limits.
- Automatic UTM/referrer capture and local lead scoring so entries are grouped as cold, warm, or hot without sending data to an external service.

Privacy
Submissions are stored in your WordPress database. If enabled per form, IP address, user agent, UTM values, and referrer URL are stored with the entry. The lead score is calculated locally on your site.
