This plugin contains helper classes used to build custom forms. It’s built by Tailored Media for use on our sites, but anyone is welcome to use it.
NOTE: I would discourage installing this on new sites. With Gutenberg & form builder plugins, everything can be done better.
This plugin comes with a basic contact form. You can write additional plugins to extend & create more forms. If you are not comfortable writing PHP code, then this plugin is probably not right for you.
It also contains some other shortcode helpers for Google Maps, jQuery UI Tabs, and Page Content.
Shortcodes
This plugin also includes some shortcodes that we tend to use a lot.
[tabs]
This will apply formatting and javascript to implement jQuery UI Tabs. To use, simply wrap all of your tabbed content in [tabs] … [/tabs] shortcodes. Each H2 element will be a new tab. Some basic CSS is included, and you can write your own in your theme file to customise the look.
[pagecontent id=”1″]
Sometimes you need to include the same bit of content in many places on your site. To save time, this shortcode will let you include the content from one page in many places. Just use the shortcode, and provide the ID of the page you want to include. Eg, [pagecontent id=”3″] will insert all content from the page with ID = 3. Only the content is included by default; use [pagecontent id=”3″ include_title=”yes”] if you also want the page title, which is added as an H2 heading with the class ‘pagecontent_title’.
Note that a page is only included if the person viewing it would be allowed to read it. Published pages are included for everyone; drafts, private, scheduled and trashed pages are only included for logged-in users with permission to read them.
[googlemap address=”123 somewhere street, Kansas”]
To embed a Google Map iframe, use this shortcode. Google will geocode your address to determine where the pin goes. You can also specify width, height, and zoom. You can also provide ‘class’ to set a CSS class on the iframe element.

The plugin contains two forms: a contact form, and a sample form. Have a look at the source code to see how to write your own forms. If you’re not comfortable writing PHP code, this plugin is probably not the best choice for you.

Security release. Fixes an attribute-injection XSS in the form shortcodes, restricts the Embed JS tool to users who may publish unfiltered HTML (this changes who can use it on multisite), and stops non-administrators changing form settings. Recommended for all users.

- Security: fix a stored XSS in the form shortcodes. The
classattribute (eg, [ContactForm class=”highlight”]) was written into the form tag unescaped, so any user who can add a shortcode could break out of the attribute. Class names are now sanitised and the form tag is escaped on output. Reported via Patchstack. - Security: the Embed JS tool now requires the
unfiltered_htmlcapability rather thanmanage_options. The code it stores is printed to the page unescaped, and on multisite a site Administrator hasmanage_optionswithout being trusted with raw HTML. Embed code is also recorded against the user who saved it and is only printed while that user still holds the capability, and the meta value is now protected so it cannot be set through the Custom Fields box or XML-RPC. - Security: the form settings page was available to anyone with
edit_posts, meaning contributors and authors could change the notification email address and the messages printed on the front end. Viewing logged enquiries is unchanged; saving and viewing settings now requiresmanage_options, and settings values are escaped when printed in the admin. - Security: form error and success messages are filtered with wp_kses_post() before being printed on the front end, and the submitted value echoed back by the email-format check is now escaped as well.
NOTE: embed code saved before this release is no longer printed on multisite, or on any site that defines DISALLOW_UNFILTERED_HTML, until a user who holds unfiltered_html (a Super Admin on multisite) opens the page and saves it again. The Embed JS box shows a warning when this applies.

- Security: fix a stored XSS in the [tabs] shortcode. Tab content written by users without the
unfiltered_htmlcapability is now filtered through wp_kses_post() after HTML reconstruction. Reported via Patchstack. - Security: fix a stored XSS in the [googlemap] shortcode. The class, width, height and zoom attributes were written into the map markup unescaped; they are now escaped and coerced on output.
- Security: fix content disclosure in the [pagecontent] shortcode. It would include any post ID regardless of status, exposing draft, pending, private, scheduled, trashed and password-protected content to visitors. Published posts are included as before; anything else now requires the viewer to have permission to read it.
- [pagecontent] no longer loops if a page includes itself, and uses get_post() instead of the deprecated get_page().
- [pagecontent] now actually implements the documented include_title=”yes” option, which was being parsed but never used. The title is added as an H2 with the class ‘pagecontent_title’. The readme previously described this option backwards.
